Provenge: survival benefit did not remove commercial constraints Clarus Biotech Review Dendreon’s 2010–2014 record separates clinical value, operating revenue, and financing resilience. Sipuleucel-T is a useful corrective to the idea that clinical success automatically produces commercial resilience. In the 512-patient IMPACT trial, median overall survival was 25.8 months with sipuleucel-T and 21.7 months with control. The 4.1-month difference was a trial result in a defined metastatic prostate-cancer setting, not a revenue forecast. [1] ## The operating result used a different denominator Dendreon's March 2014 earnings release reported 2013 net product revenue of $283.7 million, compared with $325.3 million in 2012. It also reported a 2013 net loss of $296.8 million. These company-level amounts should not be relabeled as treatment price, trial expense, or cost per patient. [2] ## Time exposed the financing constraint The company's subsequent filing records voluntary Chapter 11 petitions on November 10, 2014. That corporate outcome does not erase the trial's survival result or mean the product had no use. It does show why a product opportunity and the financial structure carrying it must be assessed separately. A complete causal account would require more than these three public observations. [3] ## Replace the headline market with a service funnel For a patient-specific therapy, model clinical suitability, referral, collection, manufacturing capacity, scheduling, reimbursement, initiation, and completion. A broad disease count omits the operations needed to serve an eligible patient. Each conversion should have a denominator and a reason for attrition rather than inherit an optimistic percentage from a category forecast. ## Set a commercial inflection gate around resilience A leadership team should review completed patient pathways, utilization of capacity, cash collection, and downside runway alongside clinician interest. Revenue growth can be real without funding the installed operating base or debt obligations. The Provenge example makes that distinction concrete: a supported clinical proposition, realized revenue, and a durable enterprise are related but separate achievements.
